The successful candidate:

The Licensed Social Worker will support the hospital's mission and vision by providing social work services to patients and their families, helping them manage and cope with the psychosocial needs and challenges associated with illness. The Social Worker assists with communication, offers counseling, navigates systems of care, provides resources, and aids in discharge planning. Additionally, they work to alleviate the stress experienced by families dealing with cancer.

  • This position will report to the Manager of Behavioral Health and work closely alongside with the Director of Patient Care Services

  • The perfect candidate will possess solid experience Bone Marrow Transplant/Stem Cell/Hematology experienceand have obtained a Master's degree from a program accredited by the Council on Social Work Education (CSWE)

Essential Job Duties:

  • Conducts social work assessment and implements a plan of care utilizing psychotherapeutic intervention for appropriate referrals.

  • Addresses needs of both patient and family through counseling, education and community resource linkage, serving as an advocate for patients/families in regard to decisions affecting the plan of care.

  • Provides crisis intervention including adjustment to hospitalization, disability, chronic illness, mental health illness, abuse, and financial counseling including assistance with applications for disability.

  • Conducts comprehensive discharge needs assessment. Initiates and facilitates referrals for home healthcare, infusion therapy, hospice, other care facilities, medical equipment and supplies.

  • Facilitates transfers to other facilities. Maintains open communication with patient, family and the multi-disciplinary team. Documents all consults and interventions in the electronic medical records in a timely manner.

  • Works collaboratively and maintains active communication with physicians, patients, families and other healthcare team members including external health care agencies to facilitate the plan of care in a courteous, professional demeanor.

  • Attends weekly interdisciplinary team meetings and comfort rounds. Documents all consults and interventions in the electronic medical records in a timely manner.

  • Assists with “hand-offs” by assuring plans are communicated to the health care team.

  • Ensures that follow-up appointment with primary physician at COH is made and patient is informed; calls patients after discharge to ensure discharge transition was smooth and proactively identifies obstacles that may delay treatment progress.

  • Ensures the safety of patients in guest rooms, outside hotels, and in the home.

  • Arranges family/caregiver and/or sitters to assist in the care of the patient to provide a safe environment.

  • Effectively manages time for maximum productivity and submits recorded productivity monthly.

Minimum Experience

  • Ability to assess the bio-psycho-social needs by evaluating roles, relationships, and resources and their impact on the patient’s health care

  • Ability to provide ongoing monitoring, counseling and/or linkage with community resources for patients and or family’s based on assessed needs

  • Ability to ensure that documentation of the Social work assessment plan, intervention and evaluation are accurate, complete and timely

  • Ability to ensure that discharge planning is appropriate, timely and communicated to the patient, family and health care team

  • Ability to implement appropriate treatment plans utilizing relevant psychotherapeutic interventions

  • Knowledge in the care and decision making for the oncology patient population and seeks guidance appropriately

Required Experience

  • Masters of Social Worker required from a program accredited by the Council on Social Work Education (CSWE)

  • Must possess experience Bone Marrow Transplant/Stem Cell/Hematology experience

  • Licensed candidates are strongly preferred, however will consider skilled candidates who will have fulfilled licenses required within two years

  • Three years of medical social work experience required

  • Candidate should be knowledgeable with federal and state resources

  • An active member of the National Association of Social Workers or Association of Oncology Social Work is preferred

City of Hope employees pay is based on the following criteria: work experience, qualifications, and work location.

Salary / Pay Rate Information:
Pay Rate: $28.58 - $47.04 / hour

The estimated pay scale represents the typical [salary/hourly] range City of Hope reasonably expects to pay for this position, with offers determined based on several factors which may include, but not be limited to, the candidate’s experience, expertise, skills, education, job scope, training, internal equity, geography/market, etc. This pay scale is subject to change from time to time.
